Diabetes Treatment Plan

 

A diabetes diagnosis inevitably means a change in lifestyle. Since we are creatures of habit, this change requires the support of family and friends. The caring Associates at the Centegra Diabetes Center help you and your loved ones adjust to your new lifestyle.  We provide you with all the information you need to keep you educated about your health and understand that you want to know what kind of treatment you will receive and how it will affect you.

First Steps
If you think you have diabetes, you may not know where to go or who to contact to get a diagnosis. At Centegra we can help you find a physician or process your referral  so you can begin Diabetes Self-Management Training as soon as possible. There are some requirements before your education starts with the Centegra Diabetes Center:

  • You must have an order from a physician.
  • Ask your doctor for your diagnosis or diagnosis code. You will need this information when you check with your insurance company.
  • If you do not have a physician, visit our Find a Doctor page on Centegra.org
  • If you already have a physician, call your doctor and  ask the office staff to  fax your referral to the Diabetes Center at (815) 356-2385.

What to expect after Diagnosis
Once you or a loved one are diagnosed with diabetes, it is important to take the necessary steps to achieve a healthy lifestyle. At Centegra we provide consultations with certified diabetes educators and registered dietitians. We have a proven education program that helps you understand the changes you need to make to reach your goals. We understand this may be an overwhelming process, but with the help of our caring Associates, you will find a treatment plan that works for you.

  • You will have an individual consultation with our educators who will help you understand the education process. We encourage you to bring a spouse, close friend or relative to learn with you during this session.
  • We offer Diabetes Self Management Training (DSMT) classes that teach patients how to manage their diabetes. It focuses on the critical self care habits that you will need to be healthy and fully enjoy life.
  • Medical Nutrition Therapy (MNT) is an essential part of diabetes care provided by a registered dietitian to develop healthy eating habits. Registered dietitians counsel on behavioral and lifestyle changes.
  • If you complete all three classes, you may participate in the Centegra WellBridge program: a three-month membership to Centegra Health Bridge Fitness Center.
  • Ongoing support groups continue after the classes are over. The support groups are safe places where diabetes patients  can discuss their experiences while offering support and advice for others in similar situations.

Diabetes Self-Management Training
The goal of our self-management class is to educate you about diabetes and ways to aggressively maintain or keep your cholesterol, blood pressure and blood sugar managed so there are no complications after diagnosis. These classes are taught by registered dietitians and nurses so you understand both the nutrition and clinical changes caused by diabetes. Other topics include exercise and stress management.  For your convenience, classes are offered at morning, afternoon and evening hours to accommodate many different schedules. Each class is outlined in the following way:

1st Class
Dietitian: How to eat with diabetes – what foods affect my blood sugar?
Nurse: What is diabetes? What medications are needed?

2nd Class
Dietitian: Eating to prevent diabetes complications
Nurse: Large blood vessels and their relation to complications like heart attack and stroke

3rd Class
Dietitian: Dining out, recipe modifications, goal setting
Nurse: Small blood vessels  that include eyes, kidneys and nerves
